ASHA Selection Committee, Barrackpore Sub Division are hereby invited from the prospective and interested candidates for engagement as Accredited Social Health Activist (ASHA) on purely voluntarily basis under NHM for different areas under all the Health Sub-Centers under Barrackpore-I & Barrackpore-II Blocks under Barrackpore Sub Division,
in pursuance of the Memo No. HFW-27011/216/2018-NHM-SEC-dept. of H & FW/Pt.(1)/1962 Date 14.06.2022 of Secretary, H & FW Department, Government of West Bengal.আশা নির্বাচন কমিটি, ব্যারাকপুর সাব ডিভিশন আগ্রহী প্রার্থীদের থেকে ব্যারাকপুর - I এবং ব্যারাকপুর - II-এর অধীনে সমস্ত স্বাস্থ্য উপকেন্দ্রের অধীনে NHM-এর অধীনে সম্পূর্ণ স্বেচ্ছায় ভিত্তিতে স্বীকৃত আশা কর্মী [ সোশ্যাল হেলথ অ্যাক্টিভিস্ট (ASHA)] হিসাবে নিযুক্ত হওয়ার জন্য আমন্ত্রণ জানানো হয়েছে।

Eligibility Criteria for engagement to the post of ASHA:-
আশা কর্মীর জন্য যোগ্যতার মানদণ্ড :-
Qualification :- Should be Madhyamik or equivalent examination appeared, candidates possessing higher qualification are also eligible. However, in all cases (appeared and failed or passed or possessing higher qualification) only the marks obtained in Madhyamik or equivalent examination will be considered.)
যোগ্যতা:- মাধ্যমিক বা সমমানের পরীক্ষায় উত্তীর্ণ হতে হবে, উচ্চতর যোগ্যতার অধিকারী প্রার্থীরাও যোগ্য। যাইহোক, সকল ক্ষেত্রে (আদর্শিত এবং ব্যর্থ বা উত্তীর্ণ বা উচ্চতর যোগ্যতার অধিকারী) শুধুমাত্র মাধ্যমিক বা সমমানের পরীক্ষায় প্রাপ্ত নম্বর বিবেচনা করা হবে।)
Age Limit:- As on 15.06.2022 should be within the age group 30-40 years.
Age Relaxation:- In case of SC/ST candidates, the lower age limit may be relaxed to 22 years. (For all candidates born not before 15-06-1982 and not after 15-06-1992; for SC/ST candidates born not before 15-07-1982 and not after 25-07-2000).
বয়স সীমা:- 15.06.2022 অনুযায়ী বয়স 30-40 বছরের মধ্যে হতে হবে।
বয়স ছাড় : - SC/ST প্রার্থীদের ক্ষেত্রে, নিম্ন বয়সের সীমা 22 বছর পর্যন্ত | (যে সকল প্রার্থীর জন্ম 15-06-1982-এর আগে নয় এবং 15-06-1992-এর পরে নয়; 15-07-1982-এর আগে এবং 25-07-2000-এর পরে নয় এমন SC/ST প্রার্থীদের জন্য৷
Others Criteria:-
1) Should be a married/divorced! widowed woman.
2)Should be a permanent resident of the same service area for which she will be selected under the health sub-centre.
3)Preference will be given to Grade I & Grade II SHG Members/ trained Dais/Link workers, keeping the selection criteria fixed.
4)Areas where the majority of the population belongs to SC/ST, preference will be given to candidates from that category, keeping the selection criteria fixed by the Sub Divisional Level ASHA Selection Committee. (Memo No. HFW-27011/375/2018-NHM SEC-Dept. of H&FW-Par-II-3 692) Date: 03-12-2021).
অন্যান্য :-
1) বিবাহিত / তালাকপ্রাপ্ত / বিধবা মহিলা হতে হবে ।
2) একই পরিষেবা এলাকার স্থায়ী বাসিন্দা হতে হবে যার জন্য তাকে স্বাস্থ্য উপকেন্দ্রের অধীনে নির্বাচিত করা হবে।
3) বাছাইয়ের মানদণ্ড ঠিক রেখে গ্রেড I এবং গ্রেড II SHG সদস্যদের/ প্রশিক্ষিত Dais/Link কর্মীদের অগ্রাধিকার দেওয়া হবে।
4) যে অঞ্চলে জনসংখ্যার অধিকাংশই SC/ST-এর অন্তর্গত, উপ-বিভাগীয় স্তরের আশা নির্বাচন কমিটির দ্বারা নির্ধারিত নির্বাচনের মানদণ্ডগুলি রেখে সেই বিভাগের প্রার্থীদের অগ্রাধিকার দেওয়া হবে। (মেমো নং HFW-27011/375/2018-NHM SEC-Dept. of H&FW-Par-II-3 692) তারিখ: 03-12-2021)।
How to apply:
Prospective and interested candidates should fill up prescribed application form (Annexure A) in her own hand writing with blue/black pen and enclose the following document:
1) Birth Certificate or Admit Card of Madhyamik of equivalent Examination (self attested photocopy) as proof of age.
2) Voters Identity card (EPIC), Aadhar Card and Ration Card (Self attested photocopy) as proof of local residence.
3) Mark sheet of Madhyamik or equivalent Examination (Self attested photocopy), even in case of failure in exam.
4) Self Attested copy of Marriage Registration Certificate in case of married women or self attested copy of Decree of Divorce in case of Divorced women or self attested copy of Death Certificate of Husband in case of widow.
- In case of above mentioned Certificates (Marriage Registration Certification, Decree of Divorce) are not available, Self Declaration regarding marital status certified by E.A/Secretary of concerned G.P. and Health Supervisor of concerned G.P in Annexure B.
5) If applicable, SC/ST Certificate from competent authority (Self attested photocopy).
6) If applicable, in case of Grade I & Grade II SHG Members certificate from BDO, Barrackpore-I & Barrackpore-II and in case of trained Dais/Link workers, certificate form BMOH, Barrackpore-I & Barrackpore-II has to be submitted along with application form.
7) Two (02) copies of identical recent Passport size colored photo, one for application form and another for Admit Card.
8) A blank envelope (Size: 23cm X 10cm) with postage stamp of Rs. 5/- affixed and the full postal address of the Candidate written on.
•. The envelope containing the Application Form and enclosures should be superscripted as
"APPLICATION FOR ENGAGEMENT AS ASHA, name of the sub-center applied for and name of Service area " and should be addressed to and to be submitted preferably by hand or via Speed Post or Registered Post within 03:00 pm on 26.07.2022
